RESUMO

The effective relief of postsurgical pain in patients undergoing knee arthroscopy is important to allow the initiation of activities of daily living. The objective of this study is to demonstrate the analgesic efficacy of dexmedetomidine as an adjuvant added to ropivacaine by the intra-articular route. METHOD: Seventy patients underwent knee arthroscopy which were randomly assigned into two groups (n=35). The RD group received ropivacaine 1.5mg/kg plus dexmedetomidine 1µg/kg intra-articularly. Group R received ropivacaine 1.5mg/kg intra-articularly. The analgesic effect was evaluated by measuring the intensity of pain (VAS score) and the duration of analgesia. RESULTS: A longer duration of the analgesic effect was observed in the RD group (655min) compared to the R group (318min) being statistically significant (p=0.03). CONCLUSION: Dexmedetomidine as an adjuvant to intra-articular ropivacaine improves the quality and duration of postoperative analgesia in patients undergoing knee arthroscopy.

Artisanal and Small-Scale Gold Mining (ASGM) represents a significant source of anthropogenic mercury emissions to the environment, with potentially severe implications for avian biodiversity. In the Madre de Dios department of the southern Peruvian Amazon, ASGM activities have created landscapes marred by deforestation and post-mining water bodies (mining ponds) with notable methylation potential. While data on Hg contamination in terrestrial wildlife remains limited, this study measures Hg exposure in several terrestrial bird species as bioindicators. Total Hg (THg) levels in feathers from birds near water bodies, including mining ponds associated with ASGM areas and oxbow lakes, were analyzed. Our results showed significantly higher Hg concentrations in birds from ASGM sites with mean ± SD of 3.14 ± 7.97 µg/g (range: 0.27 to 72.75 µg/g, n = 312) compared to control sites with a mean of 0.47 ± 0.42 µg/g (range: 0.04 to 1.89 µg/g, n = 52). Factors such as trophic guilds, ASGM presence, and water body area significantly influenced feather Hg concentrations. Notably, piscivorous birds exhibited the highest Hg concentration (31.03 ± 25.25 µg/g, n = 12) exceeding known concentrations that affect reproductive success, where one measurement of Chloroceryle americana (Green kingfisher; 72.7 µg/g) is among the highest ever reported in South America. This research quantifies Hg exposure in avian communities in Amazonian regions affected by ASGM, highlighting potential risks to regional bird populations.

BACKGROUND: Few treatments are currently available for amyotrophic lateral sclerosis (ALS). A combination of lithium carbonate and valproic acid (VPA-Li) was shown to inhibit motor neuron death and delay disease progression. METHODS: Outpatients with a typical ALS presentation were enrolled in a randomized, placebo-controlled trial to assess the efficacy of orally administered VPA-Li. Changes in a functional scale score (ALSFRS-R) and survival rate were chosen as primary outcome variables. Secondary outcome variables included BMI, respiratory monitoring, quality of life, and a global impression of the treatment. RESULTS: Out of 42 patients enrolled, 20 individuals receiving VPA-Li and 18 on placebo treatment were included in the final analysis. Forty-five percent of patients receiving VPA-Li completed the trial, whereas only 22.22% of patients in the placebo group attended the final visit 18 months later (P = 0.09). Major changes in the ALSFRS-R score were observed, including a decrease of 1.195 points/month in the placebo group (95% CI: 0.7869-1.6031) and of 0.5085 under VPA-Li treatment (95% CI: 0.2288-0.7882) between months 6 and 14. Adverse events included bad mouth taste, constipation, and anorexia. Survival rate, body weight, and quality of life were positive outcomes by the end of the trial despite a high sample reduction, especially in the placebo group. The inclusion of 212 subjects in each group would confirm these differences. CONCLUSIONS: Combined VPA-Li treatment associated with slower ALS progression and better secondary outcomes. This dual treatment overcame the futility threshold and merits further investigation in ALS.

BACKGROUND: Little is known about the neurotoxic effects of chronic exposure to airborne Mn once exposure has been reduced. The environmentally exposed and the reference adult populations evaluated in 2002 were followed, after an environmental management program (EMP) was implemented to reduce the exposure in a mining district in Mexico. OBJECTIVE: The aim of this study was to compare the association between exposure to Mn and neurocognitive performance in environmentally exposed and reference groups of adults before and after EMP implementation. METHODS: In 2013, the same battery of neurocognitive tests used in the initial study (2002) was applied to 58 adults exposed to airborne Mn and 30 adults from the reference community. A cumulative exposure index (CEI) was estimated for the study population before and after the EMP. Categorical outcomes were analyzed using logistic regression, and the resulting ORs were compared between studies. Continuous outcomes were analyzed using linear regression. All models were adjusted for age, years of education, socioeconomic status and blood lead levels. RESULTS: Exposed adults from the post-EMP study showed an improvement in fine motor and verbal regulation of motor skills (OR < 1) compared to the exposed adults from the pre-EMP study (OR > 1). In both pre- and post-studies, the exposed adults showed a deterioration in their dynamic organization of motor activity compared to the reference group (p < 0.05); however, they showed no significant change in attention and working-memory performance. DISCUSSION: After four years of a significant reduction in airborne Mn levels resulting from EMP implementation, chronically exposed adults showed an improvement in fine motor and verbal regulation of motor skills; however, the remaining areas of their motor and cognitive functions remained impaired.

Introducción: el papel del farmacéutico comunitario ha adquirido mayor relevancia en el contexto de las restricciones derivadas por la pandemia de COVID-19. Objetivo del estudio: identificar el resultado de la indicación farmacéutica ante la consulta por diarrea o estreñimiento durante este período.Material y métodos: estudio observacional, transversal, multicéntrico y nacional. Se invitó a participar a los farmacéuticos comunitarios registrados en la base de datos Medynet. Los seleccionados se estratificaron por comunidades autónomas y población rural o urbana. La población de estudio fueron los pacientes que acudieron a las farmacias comunitarias desde enero a junio de 2021 por estreñimiento o diarrea. Resultados: un total de 121 farmacéuticos de un máximo establecido de 120 reclutaron a 849 pacientes (49,7 % consultaron por diarrea y un 50,3 % por estreñimiento). Un 83,5 % de farmacéuticos consideraba haber atendido un mayor número de consultas por síntomas digestivos, pero sin llegar a ser el doble que antes de la pandemia. En los pacientes con diarrea, dieta (76,9 %) y suero oral (64,2 %) fueron las indicaciones más frecuentes, mientras que en los pacientes con estreñimiento destacaron el aumento de la ingesta de agua (86,9 %), la dieta (84,1 %) y la indicación de algún medicamento de prescripción libre (68,1 %). 4 de cada 5 consultas farmacéuticas por diarrea o estreñimiento fueron resueltas en la farmacia comunitaria sin precisar derivación al médico.Conclusión: el presente estudio refuerza el papel destacado del farmacéutico comunitario como profesional de primera línea, realizando indicaciones farmacéuticas personalizadas a pacientes con diarrea o estreñimiento, contribuyendo de forma sustancial a la eficiencia y sostenibilidad del sistema sanitario durante la pandemia de COVID-19. Introducción: la infección por meningococo del serogrupo B puede provocar enfermedad meningocócica invasiva, con un 20-30% de secuelas y hasta un 10% de mortalidad. Material y métodos: estudio observacional, descriptivo y retrospectivo de vacunación frente al meningococo del serogrupo B en la población pediátrica del Sector I de Zaragoza desde octubre de 2015 hasta diciembre de 2019. Se estudió: edad de inicio de la vacunación, edad a la fecha de la primera dosis (≤3 meses, 4-11 meses, 12-23 meses, 2-9 años, 10-16 años), sexo, centro de salud (CS) y número de dosis recibidas. Resultados: se vacunó a 11 776 pacientes, de los cuales un 51,6% fueron varones. Presentaron una edad media de inicio de vacunación a los 5,0 ± 4,4 años y 2,2 ± 0,6 dosis recibidas. La distribución del total de vacunados fue muy variada, con una diferencia del 17,8% entre el CS con más vacunados y el CS con menos vacunados. El 0,7% recibieron primera dosis en 2015, el 23,8% en 2016, el 38% en 2017, el 26,7% en 2018 y el 10,8% en 2019. El 12% tenía ≤3 meses al inicio de la vacunación, el 11,5% tenía 4-11 meses, el 6,7% tenía 12-23 meses, el 50,4% 2-9 años y el 19,5% 10-16 años, existiendo diferencias en relación con la fecha de primera dosis (p = 0,000). El 2017 cuenta con mayor incidencia de vacunación global (12,2%), aunque en lactantes fue superior en 2018 (42,1%) y en los grupos de 2-9 años y adolescentes en 2017: el 15,8 y el 5,4% respectivamente. La incidencia global acumulada fue 32,5%, siendo en lactantes de 133,5%. Conclusiones: a pesar de las prometedoras cifras de incidencia acumulada, encontramos numerosas diferencias de vacunación entre grupos de edad y CS, por lo que resulta interesante la vacunación sistemática y financiada de meningococo B (AU)


Introduction: infection by serogroup B meningococcus can cause invasive meningococcal disease, with development of sequelae in 20-30% of cases and a mortality of up to 10%.Material and methods: observational, descriptive and retrospective study of vaccination against serogroup B meningococcus in the paediatric population of health sector I of Zaragoza between October 2015 and December 2019. We analysed the age at primary vaccination, age group at time of first dose (≤3 months, 4-11 months, 12-23 months, 2-9 years, 10-16 years), sex, primary care centre (PCC) and number of received doses.Results: 11 776 patients were vaccinated, of who 51.6% were male. The mean age at initiation of vaccination was 5.0 ± 4.4 years, and they received a mean of 2.2 ± 0.6 doses. The distribution of vaccinated patients by PCC was heterogeneous, with a difference of 17.8% between the centre with the most vaccinated patients and the centre with the least. Of all patients, 0.7% received the first dose in 2015, 23.8% in 2016, 38% in 2017, 26.7% in 2018 and 10.8% in 2019. Twelve percent were aged 3 months or less when they received the first dose, 11.5% 4-11 months, 6.7% 12-23 months, 50.4% 2-9 years and 19.5% 10-16 years, with differences based on the date of the first dose (p = 0.000). The highest frequency of overall vaccination corresponded to 2017 (12.2%), although in children under 2 years it was higher in 2018 (42.1%) and in children aged 2-9 years and adolescents it was highest in 2017: 15.8% and 5.4%, respectively. The cumulative frequency of vaccination was 32.5% in the overall sample and 133.5% in the group aged less than 2 years.Conclusions: although we found promising cumulative vaccination rates, there were numerous differences in vaccination between age groups and PCCs, which is why publicly funded routine vaccination against meningococcus B is worth contemplating. El eccema coxsackium es una dermatosis infecciosa caracterizada por lesiones papulovesiculosas, eccematosas e incluso costrosas de predominio en extremidades, nalgas y región perioral. Suele aparecer en pacientes con afectación cutánea previa, como es el caso de la dermatitis atópica de los niños. El germen causante más frecuentemente aislado es el Coxsackie A6. Está considerado como una forma atípica de la enfermedad mano-pie-boca y es importante un correcto diagnóstico diferencial para evitar tratamientos innecesarios (AU)


Eczema coxsackium is an infectious dermatosis characterized by papulovesicular, ezzematous and even crusty lesions predominantly on the extremities, buttocks and perioral region. It usually appears in patients with previous skin involvement, as in the case of atopic dermatitis in children. The most frequently isolated causative germ is Coxsackie A6. It is considered an atypical form of Hand, Foot and Mouth Disease and a correct differential diagnosis is important to avoid unnecessary treatments. (AU)

BACKGROUND: WHO drug-resistant TB (DR-TB) treatment recommendations now emphasize all-oral regimens, recommending against certain injectable agents and deprioritizing others due to inferior safety and efficacy. Despite increasing focus on patient-centered care, we are not aware of systematic attempts to qualitatively document patients' perspectives on injectable agents. This may inform implementation of WHO guidelines, emphasizing the importance of consultation with affected communities. METHODS: Testimonies were provided by TB survivors who experienced hearing loss from treatment with injectable agents. Testimonies were submitted in writing in response to minimal, standardized, open-ended prompts. Participants provided a signed consent form (with options to participate anonymously or as a named co-author), and later gave input into the overall shape and recommendations of the article. RESULTS: Fourteen TB survivors in 12 countries contributed testimonies. The following common themes emerged: lack of access to appropriate testing, information, treatment, or a collaborative treatment environment; the power of supportive care and social environments; stigma and isolation from TB treatment itself and resultant disability; and inaccessibility of cochlear implants. CONCLUSIONS: Survivor testimonies indicate strong preferences for avoidance of injectable agents, supporting rapid implementation of revised WHO guidelines, as well as for quality and supportive care for both TB and disabilities.


CONTEXTE: Les recommandations de l'OMS pour le traitement de la TB pharmacorésistante (DR-TB) mettent désormais l'accent sur les schémas thérapeutiques entièrement par voie orale, préconisant de ne pas utiliser certains agents injectables et de ne plus donner la priorité à d'autres en raison d'une innocuité et d'une efficacité inférieures. Malgré l'attention accrue portée aux soins centrés sur le patient, nous ne connaissons aucune étude systématique ayant cherché à documenter de manière qualitative le point de vue des patients sur les agents injectables. Ce travail pourrait guider la mise en place des directives de l'OMS, en mettant l'accent sur l'importance de consulter les communautés concernées. MÉTHODES: Des personnes ayant survécu à une TB et ayant connu une perte d'audition due à un traitement par agents injectables ont apporté leurs témoignages. Les témoignages ont été soumis par écrit en réponse à des questions courtes, ouvertes et standardisées. Les participants ont signé un formulaire de consentement (avec possibilité de participer de manière anonyme ou en tant que coauteur nommé) et ont ensuite contribué au format général et aux recommandations de l'article. RÉSULTATS: Quatorze personnes ayant survécu à une TB provenant de 12 pays ont apporté leur témoignage. Les thématiques suivantes ont été fréquemment mentionnées : manque d'accès aux tests, informations et traitements appropriés ou à un environnement thérapeutique collaboratif ; importance des soins de soutien et de l'environnement social ; stigmatisation et isolement dus au traitement antituberculeux et handicaps qui en résultent ; et inaccessibilité aux implants cochléaires. CONCLUSIONS: Le témoignage des personnes ayant survécu à une TB indique qu'elles préfèrent nettement éviter les agents injectables, allant ainsi dans le sens d'une mise en place rapide des directives révisées de l'OMS, et qu'elles préfèrent des soins de qualité et de soutien pour la TB mais aussi pour les handicaps qui en résultent.

As managers and decision makers evaluate pollutant risk, it is critical that we are able to measure an assessment of the injury. Often, these estimates are difficult to determine for benthic organisms, so in 2017 a chronic polychlorinated biphenyl (PCB) sediment dose-response model to predict benthic invertebrate injury was proposed. Given both natural resource trustee and consultant questions following publication concerning that the aqueous chronic toxicity testing data used in the 2017 model development were primarily from the 1970s and 1980s, this follow-up short communication is meant to provide the user some additional data that are more recent. With the advances in analytical and quantitative environmental chemistry (i.e., better detection limits and congener separation), we chose to complete acute aquatic toxicity testing using 3 estuarine invertebrates and lethal endpoints (20 and 50% lethal concentrations). This acute testing was selected because chronic aquatic testing for PCBs outside of the data used in the 2017 study was not available to us. The aquatic results used in the present study were changed to sediment using equilibrium partitioning, as done in the 2017 chronic model, after using the same organic-carbon partition coefficient and total organic carbon for our equilibrium partitioning (EqP)-measured calculations. Based on these acute aquatic toxicity results and a general acute-to-chronic injury concentration ratio of approximately 10, we found that the 2017 model was valid and, hence, that a 1.0 µg/g chronic PCB sediment criterion is a reasonable estimation of potential benthic invertebrate injury. This was followed by spiked sediment tests where percent acute sediment injury was compared to the EqP-derived chronic value and the results from 2017; modest agreement is shown. Ionizing and non-ionizing radiations are part of our daily life, and when organisms are exposed to them for a long time, they may experience their lethal or sublethal effects. For this reason, technologies have been created to quantify them. In this study, Internet of Things (IoT) was used through connecting gamma meters and a low-cost UV radiation device. The validation of this structure was performed with meters calibrated in certified laboratories. The validation results matched those obtained by the other devices, with an error of 2%.

OBJECTIVE: To study the association between osteoporosis and sarcopenia and determine the prevalence of osteosarcopenia in patients who attended a rheumatology center in Ecuador. METHODS: A cross-sectional study was conducted in a population of patients who had a densitometric study. The diagnosis of sarcopenia was determined by the DXA standard gold test, screening, and conventional methods (bioimpedance, anthropometric measurements, SARC-F, muscle function, and gait test). RESULTS: A total of 92 patients were studied. The median age was 66 ± 10, 90% females. Using the criteria of SMI, 65% had sarcopenia of which 9% had only sarcopenia and 56% had osteosarcopenia; 22% had only osteopenia/osteoporosis; and 13% none of these conditions. The prevalence of sarcopenia according to handgrip strength was 60%, gait speed 45%, and SARC-F score 40%. The prevalence of osteosarcopenia according to handgrip strength was 51%, gait speed 34%, and SARC-F score 32%. Osteoporosis was associated with a higher prevalence of sarcopenia using the criteria of SMI since 40% had sarcopenia in the normal DXA group, 64% in the osteopenia group, and 76% in the osteoporosis group (p=0.017). Of the women, 69% had sarcopenia compared to 33% of the men (p=0.034). The BMI was lower in the group with sarcopenia (25.1 ± 4.1 kg/m2) compared to the group without sarcopenia (29.4 ± 4.1 kg/m2, p < 0.001). Patients with osteosarcopenia and sarcopenia had lower BMI, handgrip strength, ASM, SMI, and total-body skeletal muscle mass than those with osteopenia/osteoporosis or normal patients. CONCLUSION: 65% of the studied population had sarcopenia. It is clear that the prevalence of sarcopenia is higher in patients with greater loss of bone mass. Identifying pathways that affect both bone and muscle could facilitate the development of treatments that simultaneously improve osteoporosis and sarcopenia.

INTRODUCCIÓN: Aunque la citrulina es producida por la sintasa del óxido nítrico (NO) al activarse el receptor glutamatérgico NMDA, nitritos y nitratos (NOx) son considerados los mejores marcadores de síntesis del NO pues citrulina es también metabolizada por otras enzimas. En este estudio se realizó un análisis de correlaciones de concentraciones de citrulina y NOx en líquido cefalorraquídeo humano para evaluar la proporción en que la citrulina refleja la síntesis del NO y la neurotransmisión glutamatérgica. MÉTODOS: Se incluyeron pacientes con trastornos neurológicos agudos a los que se realizó punción lumbar (n = 240). Se determinó la concentración de NOx y aminoácidos por HPLC. RESULTADOS: NOx no fue diferente por la presencia de infección (p = 0,110) o inflamación (p = 0,349). La regresión múltiple mostró que NOx correlacionó con glutamina (r = -0,319, p < 0,001) y citrulina (r = 0,293, p = 0,005) pero no con el cociente citrulina/arginina (r = -0,160, p = 0,173). El ANCOVA confirmó que NOx está asociado con citrulina (F = 7,6, p = 0,007) pero no con el cociente citrulina/arginina (F = 2,2; p = 0,136), ni con la infección (F = 1,8; p = 0,173) o la inflamación (F = 1,4; p = 0,227). NOx no correlacionó con arginina ni con glutamato. CONCLUSIONES: Esto sugiere que citrulina refleja parte de la síntesis del NO a pesar de la contribución de otras vías metabólicas. Además, este estudio muestra que la glutamina es un modulador importante de la actividad de la NOS, y que arginina y glutamato no correlacionan con la concentración de NOx

Helicobacter pylori (H. pylori) eradication using standard triple therapy (STT) with proton pump inhibitors (PPI), amoxicillin and clarithromycin (CLA) has been the standard in Latin America. However, CLA resistance is a rising problem affecting eradication rates. Genetic polymorphisms of CYP2C19, a PPI metabolizer may also affect eradication. The primary aims of this study were to evaluate the effect of clarithromycin resistance on H. pylori eradication in a population from Santiago, and to establish the pooled clarithromycin resistance in Santiago, Chile. Symptomatic adult patients attending a tertiary hospital in Santiago were recruited for this study. CLA resistance and the polymorphisms of CYP2C19 were determined on DNA extracted from gastric biopsies, using PCR. The STT was indicated for 14 days and eradication was determined by a urea breath test 4-6 weeks after therapy. A meta-analysis of CLA resistance studies among adult residents in Santiago was performed. Seventy-three out of 121 consecutive patients had positive rapid urease test (RUT) and received STT. Sixty-nine patients (95%) completed the study. The H. pylori eradication rate was 63% and the prevalence of CLA resistance was 26%. According to the CYP2C19 polymorphisms, 79.5% of the RUT-positive patients were extensive metabolizers. Multivariable analyses showed that only CLA resistance was significantly and inversely associated with failure of eradication (OR: 0.13; 95% confidence interval [95% CI], 0.04-0.49). A meta-analysis of two previous studies and our sample set (combined n = 194) yielded to a pooled prevalence of CLA resistance of 31.3% (95% CI 23.9-38.7). Our study shows that CLA resistance is associated with failure of H. pylori eradication. Given the high pooled prevalence of CLA resistance, consideration of CLA free therapies in Santiago is warranted. We could recommend bismuth quadruple therapy or high-dose dual therapy, according to bismuth availability. Further studies need to evaluate the best therapy.

OBJECTIVE: To compare the clinical characteristics of a group of men and women with rheumatoid arthritis (RA) and determine the differences between genders. MATERIALS AND METHODS: A descriptive and comparative cross-sectional study was developed with a group of 50 men and a control group of 50 women with RA, from a rheumatology center in the city of Guayaquil, Ecuador. Data collected included clinical manifestations, comorbidities, treatment, and disease activity. Clinical and activity differences between sexes were analyzed. RESULTS: Women were more devoted to housework (66%), while men consumed more tobacco (34%) and alcohol (38%). Fatigue (60%), loss of appetite (54%), and weight loss (44%) were more common in women. No differences were found in comorbidities or treatment. Women had higher values of DAS-28 (3.4 vs 2.5), HAQ-DI (1.1 vs 0.4), ESR (33.0 vs 23.2), painful joints (8 vs 3), swollen joints (6 vs 2), and overall physician assessment (3 vs 2). CONCLUSION: The results are similar to other publications that establish that women have a more aggressive disease with greater activity of the disease and disability.

Manganese (Mn) is an essential nutrient for cellular function, but in high concentrations, it is neurotoxic. Environmental exposure to Mn has been associated with cognitive effects in children. This study aimed to assess the effect of environmental exposure to Mn on verbal memory and learning in schoolchildren residents from two municipalities in the state of Hidalgo, Mexico. Cross-sectional studies were conducted in 2006 and 2013 with a total of 265 schoolchildren of 7 to 11 years old. Children's Auditory Verbal Learning Test-2 (CAVLT-2) was used to assess verbal memory and learning. Mn exposure tertiles were defined according to hair manganese (MnH) levels determined by atomic absorption spectrophotometry. Linear regression models were used to estimate the association between MnH levels and CAVLT-2 scores. The models were adjusted by potential confounders. The lowest and highest exposure tertiles were defined below and above MnH levels of ≤ 0.72 and ≥ 3.96 µg/g, respectively. Mn exposure was significantly associated with an average of 5- to 9-point decrease in learning curves and summary CAVLT-2 scores in the highest tertile. This study adds to the evidence of decreased verbal memory and learning in schoolchildren environmentally exposed to manganese.

Under pathological conditions, nitric oxide can become a mediator of oxidative cellular damage, generating an unbalance between oxidant and antioxidant systems. The participation of neuronal nitric oxide synthase (nNOS) in the neurodegeneration mechanism has been reported; the activation of N-methyl-D-aspartate (NMDA) receptors by agonist quinolinic acid (QUIN) triggers an increase in nNOS function and promotes oxidative stress. The aim of the present work was to elucidate the participation of nNOS in QUIN-induced oxidative stress in knock-out mice (nNOS-/-). To do so, we microinjected saline solution or QUIN in the striatum of wild-type (nNOS +/+), heterozygote (nNOS+/-), and knock-out (nNOS-/-) mice, and measured circling behavior, GABA content levels, oxidative stress, and NOS expression and activity. We found that the absence of nNOS provides a protection against striatal oxidative damage induced by QUIN, resulting in decreased circling behavior, oxidative stress, and a partial protection reflected in GABA depletion. We have shown that nNOS-derived NO is involved in neurological damage induced by oxidative stress in a QUIN-excitotoxic model.
